End Of Life Care – Out Of Hours
We understand that the need for urgent care does not end when a GP surgery closes for the day. End of life care (or Palliative care) is no exception to this.
If you need to arrange end of life care for yourself, or for a loved one outside of surgery operating hours, Practice Plus Group – the new provider of Out Of Hours care in the local area – have provided the following guidance which we hope you will find useful:
