DCC Adult Social Services / Care Direct Plus  

Care Direct Plus are the team to contact if a social care needs assessment is required. The social care team will assess their care needs, in relation to how they are managing at home, and make recommendations relating to packages of care and equipment if necessary. Contact: 0345 155 1007

Intermediate Care Team
Intermediate Care is a service for people over the age of 18, who need an urgent short period of rehabilitation, treatment, or care. This may be due to a recent illness, a hospital stay, or a reduction in their ability to remain independent at home. The team can be reached on 0300 500 40 42 (Monday – Friday) or 0300 456 4622 (weekends and bank holidays).

Devon Rape Crisis and Sexual Abuse Services

This service offers confidential, professional support to survivors of rape, sexual assault and childhood sexual abuse in Devon, regardless of the time elapsed since the abuse has taken place. They can provide specialist support online, in person or by phone.

Direct support sessions are regular, specialist therapeutic sessions held either online, by phone or one of three bases.

EDMR is a specialist trauma treatment – note the waiting list is currently closed (as of March 2023).

Health Connect Coaching Programme

Volunteer, peer led, health and wellbeing coaching programme that has been designed and developed with people who have lived experience of long-term health conditions. This programme aims to support and empower individuals who are struggling to manage their condition, to build their knowledge, skills and confidence for better self-management of their health and wellbeing, in a way which matters most to them. Individuals (also known as ‘peers’) accessing the programme will be carefully matched with one of our trained Volunteer Coaches who has experience of the same or similar health condition or symptoms, someone who just understands and ‘gets it’.
