HOPE Programme at Albany – for Patients Living with Diabetes

The HOPE Programme supports people with long-term health conditions to build their confidence and learn how to manage their condition. The programme has previously helped patients with conditions such as chronic pain, fatigue, and Long COVID.

Albany will be hosting its own HOPE Programme, starting with patients at the surgery who are over 18 years of age and have been diagnosed with Diabetes.

This programme will be directed by our very own HOPE Co-Ordinator Steve, who has lived experience with Diabetes and dedicates much of his time to helping others living with the condition.

We aim to start the course towards the end of October and if you meet the criteria above, you will have received a text message or letter inviting you to express your interest or ask for more information. HOPE Co-Ordinator Steve is busy contacting those who have expressed their interest so far, but it’s not too late to get in touch if you’re still interested or would like more information.
