A Primary Care Network, or PCN, is a collaboration of local GP surgeries which work together with local services to offer more personalised, joined-up health and social care. As some resources are shared across a PCN, this allows individual GP surgeries to offer services they wouldn’t otherwise be able to offer. Across England, there are 1,250 PCNs.
Albany Surgery is a part of Newton West PCN, which is also made up of Bovey Tracey and Chudleigh Practice, and Kingskerswell and Ipplepen Practice – and is led by PCN Manager Rosie.
Each week, managers from these surgeries hold a meeting to share ideas, planning, and important information from the different surgeries.
